Behavioral Health Services
Just like with our preventative classes and Teen Court program, our therapy services continue to provide alternative options to repair harm and prevent future harmful behavior. Through our individual therapy sessions, we can now help youth understand themselves and express themselves in healthy ways. At Forge Evolution, we want to give every youth a chance to make the right choices and use their voice. Similar to our classes, our therapeutic services are available to youth in advance of an offence, offering Forge Evolution another opportunity to provide preventative interventions and a safe space for youth to communicate openly about their life challenges.

Preventative Therapeutic Services
Our therapeutic services are available to youth before an offence occurs. Read below on how to get started.
- Come in for a Behavioral Health Assessment with our mental health professional who will provide a plan of recommended sessions.
- Schedule your sessions and set up payment in a way that works best for you. Payment plans are available and we accept Medicaid.
- Complete your individualized sessions, learning coping skills, identifying goals, and building self-esteem.
For Teen Court Participants
Below is the typical trajectory of a Teen Court participant who gets involved with our Behavioral Health Services.
- Attend your Peer Panel where you may receive a recommendation to participate in our Behavioral Health Services.
- Schedule your sessions with your case manager and set up payment in a way that works best for you. Payment plans are available and we accept Medicaid.
- Complete your individualized sessions, learning coping skills, identifying goals, and building self-esteem.
- Fun fact: One hour of therapy with Forge Evolution can count as one hour of your required community service.

No, any one is allowed to participate in our Behavioral Health Services.
TEEN COURT CLIENTS
Clients participating in behavioral health services that are Court Ordered through Teen Court for a minimum of 6 weeks will be at the cost of $60.00 per session. Cost can be covered if the defendant has Medicaid or other insurances accepted by the organization. If the defendant is to use insurances accepted by the organization, the defendant is responsible for any co-pays. A sliding scale will be available to defendants (if needed) and addressed with the therapist prior to services beginning.
FORGE EVOLUTION CLIENTS
Clients participating in behavioral health services that are requesting therapeutic services and will be paying with cash, will fall under our cash pay clients. If our clients have Medicaid, we will bill Medicaid for services.
MENTAL HEALTH ASSESSMENTS ONLY
Clients participating in mental health evaluations that have been requested through the court will cost $200.00. Clients will receive a complete evaluation with recommendations given to the court (if applicable).
CASH PAY CLIENTS
Our standard out-of-pocket fee is $60.00 for a 50-minute session. Forge Evolution offers a sliding scale that can be discussed with the therapists. Forge Evolution requests payment by debit/credit card or cash at the time service is rendered. If any payments are returned due to insufficient funds, a $25 fee will be charged to you. As a general policy, the organization requests that clients pay directly for fees unless otherwise specified. Meetings with auxiliary medical or legal consultants, DHS, Probation or Parole either by phone or in person and report writing will be billed at the standard fee of $85 per hour. If payment is in arrears more than 90 days and a fee payment schedule cannot be agreed upon, your account will be turned over to a collection agency, an attorney, or small claims court.

You are welcome to continue. You do not have to be in Teen Court to continue with Behavioral Health Services.
Yes, we do take Medicaid and CHP plus. If you do not have insurance we offer services on a sliding scale.
If you are unable to keep an appointment, please notify your therapist as soon as possible. If you cancel an appointment without giving 24 hours’ notice, you will be billed for half of the standard fee. If you are a no show/no call you will be billed the full fee for the session. A debit/credit card on file is required (if applicable) before services are rendered and will never be charged without informing you first via phone, letter, text, or email.
If you are unable to keep an appointment, please notify your therapist as soon as possible. You will receive one cancellation, any additional cancellations you will be charged a 20.00 rescheduling fee.
